The Fen Zeil's Headhunter Customization is a Galactic Seasons 11 Achievement item which can be found in Star Wars: The Old Republic.
How to get the Fen Zeil's Headhunter Customization
GALACTIC SEASON 11: Some of Fen Zeil and Amity's customizations are only available as an achievement for Galactic Season 11. The Meta-Achievement reward for this season is a 'The Penumbra Armor Set and Customizations Sets' and can be earned by completing all Seasonal Achievements. The Meta-Achievement gets you a set The Penumbra Armor Set, Amity's Penumbra Customization, Fen Zeil's Headhunter Customization and Fen Zeil's Blaster Twirler Customization. See Galactic Season 11 Achievements section. After That: It is assumed these customizations will be retired.
How to Equip a Companion Customization
Once you've got a copy of the Fen Zeil's Headhunter Customization in your inventory, you can equip it to your companion in the companion tab of the character sheet. How to Use a Companion Customization
