How to get the Hot Shot Pilot codex: Earned by completing the highest level space mission. This codex is from before the introduction of heroic space missions. As such, while those are currently the highest level missions, the actual mission one needs to complete is "The Impossible Sector" (Republic) and "Ascendancy Barrier" (Imperial)

Screenshots: SWTOR-Spy.com does have a gallery of images for the 'Hot Shot Pilot' codex.

Lore Entry: You earned this title by completing the highest level space mission.
